Responsibilities

  • Provide patient care coordination for Cardiology patients
  • Patients are assessed properly
  • Medications dispensed according to all established guidelines
  • Assess learning readiness & provide discharge teaching
  • Assure future tests, medication changes & instructions are understood
  • Completes required documentation for patient office visits
  • Assists Physician in follow up with patients regarding care, such as test results and plan of care
  • Triages patient care as necessary
  • Responds appropriately to needs of patients calling into clinic including prescription refills, treatment of symptoms, etc. following policies
  • Assist with prepping of charts for patients for upcoming visits by gathering information from other hospitals and clinics
  • Reviews patient charts for copies of procedures for office visit
  • Update/establish patient problem list in EMR
  • Reviews patient chart or records to update problem lists and medication list
  • Assists with daily clinic to including: rooming patients, completing labs requests, scheduling procedures, providing patient education, entering information in the EMR and completing EKG’s as needed
  • Collaborates with nursing colleagues and the interdisciplinary team to assure coordination of care.
  • Travel to outreach sites as needed.


Education
  • 2 year degree Registered Nurse, BSN preferred
  • ADN


Qualification/Skills

Experience
  • Preferred 2 years of cardiac experience

Skills
  • Commitment to the mission and values of MHI clinic
  • Demonstrated professional image via appearance and conduct; ability to communicate effectively with patients, physicians staff and public
  • Sound knowledge of CV health and related diagnostic clinical therapies, ability to teach patients, families , staff and students

Licensure/Certification
  • Current MN – RN license
  • ACLS and BLS certified
