### Summary

The incumbent functions at the highest level of clinical practice,
working independently under his/her scope of practice as defined by
the medical center to directly care for patients. This Academic
Detailer Pharmacy Program Manager/ Clinical Pharmacy Specialist
functions to initiate, modify, discontinue, & monitor medication
therapy and acts as a consultant for medication therapy management
services. This includes: designing, implementing, assessing,
monitoring, & documenting therapeutic plans.

### Responsibilities

Functions of the position include, but are not limited to, the

- Cooperate and collaborate with the VPE to align standard work
with VISN and local objectives.
- Participate in local/VISN/National workgroups.
- Develop tools (clinical guidance, policies, clinical
dashboards/reports, consults) to optimize disease management.
- The A.D. Pharmacist will be available to give lectures or
smaller in-service presentations related to the Clinical
Practice Guidelines.
- Educate providers with current updates in evidence based
- Provide guidance and methods for improving care
- Provide consultation support in the field of specialty care
assigned to the CPS
- Develop, implement, and manage programs to optimize drug therapy
costs, including the development of disease management programs.
- Identify, develop, and implement appropriate formulary programs,
outcomes, assessment, and cost-saving initiatives.
- Design and coordinate the implementation of pharmacoeconomic and
outcomes related evaluations.
- Responsible for consult responses and recommendation in assigned
project areas.
- Monitors and trends pharmaceutical costs to include reviewing
Pharmaceutical utilization to identify best practices and
opportunities for improvement.
- Conduct prospective/retrospective Medication Use Evaluations of
high-use, high costs, and/or high risk drugs to develop
corrective actions and provide education programs/interventions
that would alter aberrant prescribing or work patterns.
- Responsibility for and track cost savings achieved through
various cost-saving initiatives and implementation of clinical

PGY-1 or PGY-2 Pharmacy Residency Program.
**Specialized Experience:** Preferred 3 years Experience as
Ambulatory Care Clinical Pharmacy Specialist.
**Work Schedule:** 7:45am to 4:30pm **"Occasional Alternate Tour

- **Licensure.** Full, current and unrestricted license to
practice pharmacy in a State, Territory, Commonwealth of the
United States (i.e., Puerto Rico), or the District of Columbia.

**Preferred Experience:** Completion of PGY-1 or PGY-2 Pharmacy
Residency Program

**Grade Determinations**:
(a) **Experience:** 1 year of experience equivalent to the next
lower grade level (GS-12) and must fully meet the following KSAs:
Knowledge of professional pharmacy practice; Ability to communicate
orally and in writing to both patients and health care staff;
Knowledge of laws, regulations, and accreditation standards related
to the distribution and control of scheduled and non-scheduled drugs
and pharmacy security; Skill in monitoring and assessing the outcome
of drug therapies, including physical assessment and interpretation
of laboratory and other diagnostic parameters.

(b) **Clinical Pharmacy Specialist.** The clinical pharmacy
specialist (CPS) functions at the highest level of clinical
practice, works independently under their scope of practice as
defined by the individual medical center to directly care for
patients. A CPS plays a defined role in budgetary execution and
serves as a mid-level provider who functions to initiate, modify or
discontinue medication therapy and as a consultant for intensive
medication therapy management services. This includes, but is not
limited to, the following: designing, implementing, assessing,
monitoring and documenting therapeutic plans utilizing the most
effective, least toxic and most economical medication treatments;
helping achieve positive patient centric outcomes through direct and
indirect interactions with patients, providers, and
interdisciplinary teams in assigned areas; performing physical
assessments; and ordering laboratory and other tests to help
determine efficacy and toxicity of medication therapy. Pharmacists
assigned to this position must demonstrate the following KSAs:

(c) **Demonstrated KSAs:** The KSAs shown below apply to all
assignments at this grade level:
1. Ability to communicate orally and in writing to persuade and
influence clinical and management decisions.
2. Expert understanding of regulatory and quality standards for
their program area.
3. Ability to solve problems, coordinate and organize
responsibilities to maximize outcomes in their program area or area
of clinical expertise.
4. Expert knowledge of a specialized area of clinical pharmacy
practice or specialty area of pharmacy.
5. Advanced skill in monitoring and assessing the outcome of drug
therapies, including physical assessment and interpretation of
laboratory and other diagnostic parameters.
